4852 to be treated as a W-2. The IRS will not accept your Form 4852 as a form that shows tax liability on an income tax return. It must be filed as a substitute for Form W-2. The IRS will issue Form 4852 only when you file a Form 1040X, 1040A/B/C, or 1040SS. Complete Form 4852 to be treated as a W-2. Use the same taxpayer account, taxpayer number, and Social Security Number. If you need to adjust your tax liability, use Form 1040X, 1040A/B/C, or 1040SS to correct the tax. Do not re-file Form 4852. You must also pay the balance due on Form 4852, even if you used the incorrect taxpayer account, taxpayer number, and/or Social Security number. If you are a U.S. citizen or U.S. resident alien, you must attach the original or certified copies of Form 4852 to Form 1040X, 1040A/B/C, or 1040SS. Report any overpayment or underpayment on Form 4852. If your spouse has filed Form 1040 in his or her name as a married filing jointly but you use Form 1040 in your spouse's name, you may be able to treat Form 1040 or Form 1040A/B/C as both W-2s and 1040s, because Form 1040 does not take account of your spouse's joint earnings when it calculates taxable wages on Form 1040. If you use Form 1040, or if your Form 1040 was filed in your name by your spouse, you may not have the correct Form 1040 to show income tax on line 18 of Form 1040. To figure the correct amount of income tax due on line 18, use Form 1040A/B/C. To figure the correct amount of tax that you must pay on Form 1040SS, use Form 1040SS. If you and each of your dependents are required to file a joint tax return, you must file a combined return (in box 2, line 21 on Form 1040 or line 28 on Form 1040A/B/C). Report the joint income tax, social security tax, Medicare tax, and child and dependent care credit shown on Form 1040 in box 10.
